Does Propranolol Work for Anxiety?

Propranolol is a beta blocker that slows the heart rate, so it can ease physical signs of anxiety such as a pounding heart and shaking. It is not FDA-approved for anxiety, and the research behind this use is thin. MedlinePlus, from the National Library of Medicine, says propranolol is "also sometimes used to treat anxiety," which makes it an off-label treatment.

The most recent pooled evidence is less encouraging than its popularity suggests. A systematic review published in the Journal of Affective Disorders in 2025 screened 3,068 records and found only 10 usable studies. Five could be combined, covering 179 people in total, and they showed no evidence that beta blockers worked better than placebo or benzodiazepines for social phobia or panic disorder. This post covers what propranolol is, what the research found, the risks, and which anxiety treatments have stronger support.

What Is Propranolol?

Propranolol is a heart and blood pressure medication. MedlinePlus explains that it is used for high blood pressure, irregular heartbeat, angina, certain types of tremor and migraine prevention, and that it works by relaxing blood vessels and slowing heart rate.

Beta blockers act on the body's response to adrenaline. Anxiety often comes with a racing pulse, trembling hands and sweating, and those are the symptoms a beta blocker can dampen. It is not aimed at worry, dread or the thoughts that drive them.

That distinction shapes every other question about the drug. Propranolol may quiet the body. It is not designed to treat the anxiety disorder itself.

Is Propranolol FDA Approved for Anxiety?

No. The FDA label for extended-release propranolol lists hypertension, angina, migraine prevention and a heart muscle condition called hypertrophic subaortic stenosis. The word anxiety does not appear anywhere in the label.

Prescribing it for anxiety is therefore off-label. Off-label means a prescriber is using an approved medicine for a purpose the FDA has not reviewed. If it is suggested for you, ask what evidence supports the choice in your case.

Compare that with medications that do carry anxiety indications. The FDA label for sertraline (Zoloft), for example, includes panic disorder and social anxiety disorder. The post on medications that help with anxiety disorders covers the main options.

What Does the Research Say About Propranolol for Anxiety?

Two systematic reviews, published about a decade apart, reached the same conclusion.

The first appeared in the Journal of Psychopharmacology. Steenen and colleagues found eight trials of propranolol for anxiety disorders: four in panic disorder with 130 participants in total, two in specific phobia with 37, one in social phobia with 16, and one in PTSD with 19. They concluded that the quality of evidence was "insufficient to support the routine use of propranolol in the treatment of any of the anxiety disorders."

The 2025 review by Archer and colleagues widened the search to every beta blocker. It still found no evidence of a benefit over placebo or benzodiazepines in social phobia or panic disorder. The authors pointed out that many of the studies were small, had missing data and carried a high or unclear risk of bias, and they called for a large trial to settle the question.

Lack of evidence is not proof that a drug fails. It means nobody has run a study large enough to know. For a medication used this widely, that is a real gap.

What About Stage Fright and Performance Anxiety?

Propranolol is often discussed as something people take before a speech, an interview or a performance. Both reviews above looked at people with diagnosed anxiety disorders, so they do not answer whether a single dose helps an otherwise healthy person through a one-time event.

The logic is easy to follow. If your main problem on stage is a shaking voice and a hammering heart, slowing the heart may help you get through it. If your main problem is fear of being judged in most social situations, that pattern has a name and a set of treatments, covered in the post on how social anxiety disorder is diagnosed and treated.

Do not take someone else's propranolol for a big event. A drug that changes heart rate and blood pressure needs a prescriber who knows your medical history.

Why Is Propranolol Prescribed for Anxiety So Often?

Use has grown despite the weak evidence. The 2025 review reported that beta-blocker prescriptions for patients with anxiety rose substantially between 2003 and 2018 and that no clinical guidance covers this use. The authors work in the United Kingdom, where national guidance for anxiety does not recommend beta blockers.

A 2024 study in the British Journal of General Practice asked 17 general practitioners why they prescribe it. Many saw beta blockers as low risk. Some viewed them as an alternative to benzodiazepines that acts quickly and does not lead to dependence. Several noted that patients were more willing to try a beta blocker than an antidepressant because they did not think of it as a mental health drug.

Those are understandable reasons. They are also reasons based on convenience and perception more than on trial results, which is why the researchers called for better studies.

What Are the Risks and Who Should Avoid Propranolol?

Propranolol has warnings to take seriously. The FDA label states that patients with bronchospastic lung disease should in general not receive beta blockers because the drug may provoke a bronchial asthmatic attack.

The label also warns that beta blockers may hide early warning signs of low blood sugar, such as a fast heartbeat, which is a concern for people with diabetes. MedlinePlus advises telling your doctor if you have a slow or irregular heartbeat, heart failure, or asthma or other lung disease.

Common side effects listed by MedlinePlus include dizziness or lightheadedness and tiredness. Stopping is its own risk. MedlinePlus warns that suddenly stopping propranolol can cause serious heart problems, and the label says the dose should be reduced gradually over at least a few weeks when the drug is being discontinued.

Which Anxiety Treatments Have Stronger Evidence?

Some treatments are approved specifically for anxiety disorders. Sertraline, for one, holds FDA approval for panic disorder and social anxiety disorder, and therapy is part of care for many people. The post comparing anxiety medication and therapy looks at how the two fit together.

Panic attacks deserve their own plan. Recurrent attacks with ongoing fear of the next one may be panic disorder, explained in the post on panic disorder and how it is treated.

A beta blocker can still be part of a plan your prescriber builds. The useful question to ask is what it is meant to do for you, how you will know whether it is working, and what the longer-term treatment is. The post on what to know before starting anxiety medication has more questions to bring to an appointment.

FAQ

Is propranolol approved for anxiety?

No. Its FDA label covers heart and blood pressure conditions and migraine prevention. Use for anxiety is off-label.

Does propranolol stop anxious thoughts?

It is not designed to. Propranolol slows the heart and can reduce physical symptoms such as trembling. It does not target worry or fear directly.

Is propranolol addictive?

General practitioners interviewed in a 2024 British study viewed beta blockers as not leading to dependence, which is one reason they prescribe them. It still should not be stopped suddenly, because abrupt withdrawal can cause heart problems.

Can people with asthma take propranolol?

The FDA label says patients with bronchospastic lung disease should in general not receive beta blockers. Tell your prescriber about any lung condition before taking it.

Is propranolol safer than Xanax for anxiety?

The two carry different risks, and reviews have not found that beta blockers work better than benzodiazepines for panic disorder or social phobia. Which is appropriate, if either, depends on your health history and should be decided with your prescriber.

Can I take propranolol every day for anxiety?

Daily use is a prescriber's decision. The published reviews found too little evidence to support routine use for any anxiety disorder, so ask what the plan is and how long it is meant to last.
